It is up to the studios to get the bands permission. I have a friend and it is her full-time job clearing objects for movies and tv.

My noise pop festival poster was used in a Lindsay Lohan movie. No one ever contacted me, the festival or the 100 bands on the poster for permission. I didn't know about it til I saw it flash in the screen of the person siting next to me on the flight to my first euro flatstock.

Same here. I'm sure that 90% of the time, the management of the band is contacted first, then the artist. (American Dad, for me)

The last few times I've been approached to have my stuff in most recently a big screen movie and a tv show, they want some of the most expensive / rare posters I have. When I tell them I won't send them the actual print unless they buy one BUT I'd be happy to send a hi-res digital file, they're all of a sudden less interested.

Does anyone else seem like people are just trying to get some free stuff?

yep. a few times i've offered to give the poster for free in exchange of just getting a photo of the poster in use on set that i can put on my website ( with proper credits ) after the movies release. has never happened.
