Screen printing custom made cd boxes?

Hi guys!
Do you know anyone that would be willing to screen print on heavy paper,one color screen print and then trim/cut it in a way that would form a box when folded,in a run of 500? The measurements and all would be custom, made by us.It's for an audio series a label will release and wants a box to send out to it's subscribers so they can put inside the cds when they all come out!
Do you know of places that would do all this printing and cutting and trimming thing?
