Roller Derby poster commission

Looking for someone to do a derby poster for a 2012 bout.

Specs: 12"x18" or similar small sized format.
3 color screen printed.
Run of 50

How much would you charge for this and what kind of lead time do you need?
Also looking for printer suggestions if you don't do the printing yourself. This is for a Salt Lake City event so printers should be at least in the region.

Is there anything I've missed?
