When she says she's not a girl she's saying she's a vampire. Anything else is reading into it.

the book is not so ambiguous...

"The novel presents Eli as an androgynous boy, castrated centuries before by a sadistic vampire nobleman. The film handles the issue of Eli's gender more ambiguously: a brief scene in which Eli changes into a dress offers a glimpse of a suggestive scar but no explicit elaboration.[10] Eli's character is played by a female actress, but also tries to tells Oskar "I'm not a girl" when he asks that Eli be his girlfriend. According to an interview with the director, as the film was originally conceived, flashbacks explained this aspect in more detail, but these scenes were eventually cut."

since the scar is seen in the movie I interpreted it this way, from that wiki-

Like I was saying, in the film, Eli is a girl. Director's own words: Oskar and Eli are "mirror images of each other. She is everything he isn't. Dark, strong, brave, and a girl."

It may be in the book, but it's not in the movie.
