When she says she's not a girl she's saying she's a vampire. Anything else is reading into it.

the book is not so ambiguous...

"The novel presents Eli as an androgynous boy, castrated centuries before by a sadistic vampire nobleman. Like I was saying, in the film, Eli is a girl. Director's own words: Oskar and Eli are "mirror images of each other. She is everything he isn't. Dark, strong, brave, and a girl."

It may be in the book, but it's not in the movie.
