I had kind of the opposite reaction to the realism. For instance, "Gotham" just seemed kind of bland and banal to me. It was just Chicago. There was nothing dark or sinister about it in any way and I found it took me out of the story. You have this "dark knight" but he's in this clean, normal city.
The Joker was, indeed, spectacular. Stupid asshole Ledger had to go and kill himself.

It was a clean and normal city because the batman was working, progress was taking place the drug dealers were scared - that was the point.
Oh, and this WAS the best comic book movie ever made and one of the best action/crime thrillers ever made, EVER.
