from what i have read in the past is that geffen had this photo in their image bank and when kurt was shown it he got the idea of the dollar bill on a hook.
alex grey did the illustration within the booklet for in utero of athe muscle structure of a pregnant woman. the layout of the whole thing was done by kurt and robert fisher.
incestide is the album with a painting of two dolls that was done by kurt as well as the dolls that they were modeled after.

Yeah man, he made a fucked up sculpture of little baby dolls and stuff for the cover of inutero. And on the back cover is a collage of dieseased vaginas and slabs of meat.

incesticide was the best cover they had then "bleach". it always seemed like the artwork that cobain did himself was the best but never made it to the cover but the one.

I know this has become a legendary bit of rock art. But even tho I loved this record I even then in 91 thought it was a lame visual gag. Sorta like high-school art class kinda lame.
